Who this applies to

Pure Wellness Chiropractic operates purewellchiro.com, the five clinic domains, and a personal site for each of our doctors. All of them are covered by this policy and all of them send form submissions to the same internal system.

What we collect

Information you type in. Appointment requests, contact messages, insurance verification forms, and the site chat. Depending on the form that means your name, phone number, email address, preferred clinic, preferred day and time, how you heard about us, and anything you write in the message field. The insurance form additionally collects your date of birth, carrier name, member ID, and group number.

Information collected automatically. Standard analytics through Google Analytics (GT-5TGBWKJN) — pages viewed, approximate location, device and browser, and the site or search that referred you. If you arrive from an ad or campaign link, the campaign tags in that URL are recorded with your form submission so we know which sources actually produce patients.

Where it goes

Form submissions go into Lead OS, our internal system, and are assigned to the front desk of the clinic you selected. They are not dumped into a shared public inbox. Insurance member IDs and dates of birth are visible only to the assigned desk and are never sent by email.

We do not sell patient lists, and we do not share your information with advertisers or data brokers. Information is disclosed outside the organisation only to service providers who need it to run the sites and phones, to your insurance carrier when you have asked us to verify benefits, or where the law requires it.

What not to send us

Please do not submit full Social Security numbers, detailed medical records, or images of medical documents through the public forms or the site chat. If we need any of that, we will collect it in the office or over the phone. The chat assistant on these sites cannot take member IDs and cannot diagnose anything.

How long we keep it

Enquiries that never become appointments are kept while they are still actionable and then removed. Once you become a patient, your record falls under the clinic’s medical records retention schedule and Delaware law rather than this policy. Analytics data is retained on Google’s standard schedule.

HIPAA and clinic records

Health information created or received in the course of your care — exam findings, treatment notes, imaging, and billing records — is protected health information under HIPAA. It is governed by the clinic’s Notice of Privacy Practices, which you are given at your first appointment and can request a copy of at any time. Nothing you submit through a website form becomes part of your medical record until clinic staff enter it there.

Cookies and analytics

These sites use cookies for analytics and to remember your chat session within a single browser session. You can block or delete cookies in your browser settings; the sites will still work, and the appointment and insurance forms will still submit. If your browser sends a Do Not Track or Global Privacy Control signal, we treat that as a request not to share your information for advertising purposes — which we do not do regardless.

Your choices

You can ask us what we hold about you, ask us to correct it, or ask us to delete a website enquiry. For requests about your medical record rather than a web form, contact the clinic directly, since those requests have their own verification requirements under HIPAA.

Children

These sites are not directed at children and we do not knowingly collect information online from anyone under 13. Appointments for minors are requested by a parent or guardian, and treatment of patients under 18 requires parental consent.
